If you already have an existing account please fill out this direct deposit form and attach a voided check that has your information printed on it or attach a direct deposit letter from your prepaid card or banking institution that has your name, account and routing number typed on it. Please note: name on account must match name on file, deposit slips are not acceptable. Also, please be sure to include your consumer number and drop off, mail or fax to 1.800.359.2884
Please note: You cannot deposit your payment into an account that is not yours. You also cannot deposit payment for services in a consumer’s or surrogate’s account.

Change or Modify
If you would like to change your existing direct deposit to another account please fill out another direct deposit application form write the word "change" on the top of the form and drop off, mail or fax back to 1.800.359.2884. Please keep in mind these will be processed in the order in which they are received. Please allow ample time for changes to be made before submitting your timesheet to ensure changes will reflect on that payroll.
If you need to cancel your direct deposit please send a signed and dated letter requesting to cancel your existing direct deposit. Since we no longer issue paper checks, we cannot issue payment until new direct deposit information is in place. Please keep in mind this will be processed in the order it is received. Also, please allow ample time for changes to be made before submitting your timesheet to ensure changes will reflect on that payroll.
