Notify Tempus Unlimited of a change
In order to make sure our records are up-to-date, twice a year we send out the Change Form and Supply Request. On this form, you can notify us of a change or request supplies (ex. Timesheets, new hire paperwork, etc.). We have listed this form below:
If you do not have access to printing this form, you can always send us a signed and dated letter (and include your consumer number) with the information you would like to update, or call to request supplies. If your worker's information has changed, we must receive a signed letter from them (including your consumer number) so we can update that information.
